CERTIFICATE IN DENTAL LABORATORY TECHNOLOGY
1 YEAR PROGRAM |
- Inquire for details at the Admissions Office.
- Pay the Admissions Processing Fee (includes application form, informational materials, testing and processing fee at the Accounting Office).
- For Filipino Students : P 300.00
- For Foreign Students : US $ 32.00
- Present the Official Receipt to the Admissions Office and secure the Application Form and other requirements.
- Fill out the Application Form and attach the required documents.
- Submit completed application form with all the required documents at the Admissions Office before your entrance exam date to secure your exam permit.
- Take the exam on your scheduled date. All applicants must present their exam permits on their date of examination.

- Updated Alien Certificate of Registration (ACR) issued by the Bureau of Immigration and Deportation (BID)
- Three (3) 2” x 2” identical photographs.
- Processing Fee of P 300.00
- Immigrants and permanent resident aliens are required to submit a verified copy of their ACR.

| C. Non-Resident Foreign Students |

- Completed Application Form for Admission
- Scholastic records signed by the school head and duly authenticated by the Philippine Embassy located in the applicant's country of origin or residence
- Medical Health Certificate
- Letters of Recommendation (either from the Principal of the school or from a former teacher or professor)
- Seven (7) copies of the applicant's latest 2" x 2" photo
- Personal History statement (5 copies)
- Payment of Acceptance Fee of Fifty US dollars (US$50.00) or its peso equivalent at the prevailing exchange rate upon release of the Letter of Acceptance (LOA).
- Before the enrollment, the student is required to submit the Alien Certificate of Registration (ACR).

- Transfer Credentials/Honorable Dismissal from the school last attended
- Transcript of Records or Certification of all the subjects taken with the corresponding grades obtained
- Certificate of Good Moral Character
- Three (3) copies of the applicant's latest 2" x 2" photo
- Additional Requirement for Resident Foreign Students: Updated Alien Certificate of Registration (ACR) issued by the Bureau of Immigration and Deportation (BID)
