Post - Grad Courses:

These programs are intended for Dental Technicians, Dentists (undergraduate or graduate) who want to refresh and develop their skills in dental laboratory.)
I. Dental Porcelain Techniques (120 hrs.)
II. Beginner Course in Denture Work (72 hrs.)

Requirements:
  • Photocopy of PRC (for Dentists)
  • Photocopy of TOR or Diploma
  • One (1) pc. of 2x2 ID Picture
  • One (1) pc. Of 1x1 ID Picture

APPLICATION PROCEDURES

 
  1. Inquire for details at the Admissions Office.
  2. Pay the Admissions Processing Fee (includes application form, informational materials, testing and processing fee at the Accounting Office).
    • For Filipino Students : P 300.00
    • For Foreign Students : US $ 32.00
  3. Present the Official Receipt to the Admissions Office and secure the Application Form and other requirements.
  4. Fill out the Application Form and attach the required documents.
  5. Submit completed application form with all the required documents at the Admissions Office before your entrance exam date to secure your exam permit.
  6. Take the exam on your scheduled date. All applicants must present their exam permits on their date of examination.
 
   

I. For High School Graduates

 
  A. Filipino  
  1. Completed Application Form for Admission
  2. Form 137 or Form 138
  3. Certificate of Good Moral Character
  4. Three (3) 2” x 2” identical photographs.
  5. Processing Fee of P 300.00
  6. Photocopy of Birth Certificate (From the Local Civil Registrar)
 
   
  B. Resident Foreign Students  

The foreign students shall communicate directly with the school and comply with the following school requirements.

  1. Completed Application Form for Admission
  2. Form 137 or Form 138
  3. Certificate of Good Moral Character
  4. Updated Alien Certificate of Registration (ACR) issued by the Bureau of Immigration and Deportation (BID)
  5. Three (3) 2” x 2” identical photographs.
  6. Processing Fee of P 300.00
  7. Immigrants and permanent resident aliens are required to submit a verified copy of their ACR.
   
  C. Non-Resident Foreign Students  
  1. Completed Application Form for Admission
  2. Scholastic records signed by the school head and duly authenticated by the Philippine Embassy located in the applicant's country of origin or residence
  3. Medical Health Certificate
  4. Letters of Recommendation (either from the Principal of the school or from a former teacher or professor)
  5. Seven (7) copies of the applicant's latest 2" x 2" photo
  6. Personal History statement (5 copies)
  7. Payment of Acceptance Fee of Fifty US dollars (US$50.00) or its peso equivalent at the prevailing exchange rate upon release of the Letter of Acceptance (LOA).
  8. Before the enrollment, the student is required to submit the Alien Certificate of Registration (ACR).
 
   

II. REQUIREMENTS FOR TRANSFEREES

 

The ALPS Technology Academy, Inc., as an institution policy accepts transferees provided they completely comply with the prescribed documents and requirements, pass the TATAI-ET qualifying examination and pass the interview conducted by the Chief Instructor.

  A.  Filipino and Resident Foreign Students
  1. Completed Application Form for Admission
  2. Transfer Credentials/Honorable Dismissal from the school last attended
  3. Transcript of Records or Certification of all the subjects taken with the corresponding grades obtained
  4. Certificate of Good Moral Character
  5. Three (3) copies of the applicant's latest 2" x 2" photo
  6. Additional Requirement for Resident Foreign Students: Updated Alien Certificate of Registration (ACR) issued by the Bureau of Immigration and Deportation (BID)

  B.  Requirements for Non-Resident Foreign Students
  1. Completed Application Form for Admission
  2. Transfer Credentials/Honorable Dismissal
  3. Transcript of Records or Certification of all the subjects taken and the corresponding grades obtained
  4. Certificate of Good Moral Character
  5. Seven (7) copies of the applicant's latest 2" x 2" photo
  6. Updated Alien Certificate of Registration (ACR) or Special Study Permit (SSP) issued by the Bureau of Immigration and Deportation (BID)
  7. Payment of Acceptance Fee of Fifty US dollars (US$50.00) or its peso equivalent at the prevailing exchange rate upon release of the Letter of Acceptance (LOA).


 

III. Admission Requirements for Degree Holders

 
  1. Completed Application Form for Degree Holders
  2. Transcript of Records
  3. Two (2) copies of the applicant's latest 2" x 2" photo
